How Do You Get Item 4 To Work

Hey when i make an item 4.cfg when i try to create the item in there it never works :o can anyone please tell me how to make it work?

From my experience i've never been able to get it to work, but i've never tried editing the log servers or HG servers to tell it to load a item4.cfg..you'll have to give that a go.
